Physical Therapist Location: West Branch, MI Agency: Cynet Health Pay: $2,846 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position Job Title: Home Health Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: Monday-Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum one year experience preferred License: Currently licensed as a Physical Therapist in the state and in good standing Certifications: Current automobile insurance and valid driver’s license Must-Have: Knowledge of principles, concepts, and methodology of physical therapy appropriate for terminally ill patients. Demonstrates problem-solving and decision-making abilities. Ability to manage patient care with specific knowledge and experience in bedside nursing, symptom management, crisis intervention, and family intervention. Excellent communication skills. Ability to develop and maintain rapport with patients, families, and staff. Description: The Physical Therapist reports to the Director of Professional Services. This role is responsible for providing medically prescribed, goal-oriented therapy in accordance with the Physician Plan of Care, company policies, and professional Standards of Practice. The Physical Therapist participates in the development of patient physical therapy plans of care. Makes visits to patients for assessments and evaluations and communicates with the physician before the start of care as needed. Implements the plan of care. Documents the care and submits paperwork in an accurate and timely manner to update and maintain the patient medical records. Provides education and instruction to team members, patients, and their families. May participate in agency quality assessment performance improvement programs. Accepts other assignments deemed appropriate. Ensures the growth and profitability of the company through the responsible use of company resources and educating the community about services. Participates in regularly scheduled interdisciplinary team meetings to coordinate the care of the patient and family, exchange information, problem-solve, and receive staff support and education. Applies the policies and procedures of the agency and the rules and regulations of federal and state regulatory agencies and other certified agencies in providing care. Provides skilled restorative treatments for pain reduction, develops/restores/maintains functions, including treatments using moist heat, whirlpool, paraffin baths, gait training, ultrasound, and electrical muscle stimulation. Performs tests and measurements for range of motion using a goniometer. Instructs, establishes, and supervises exercise programs, prosthetic training, balance, coordination training, and cardiovascular rehabilitation. Instructs in home safety. Conducts initial and ongoing comprehensive assessments of patient needs, including outcome and information set assessments at appropriate time points. Evaluates outcomes of care. Directs and supervises Physical Therapist Assistants, Certified Nursing Aides, and Home Health Aides in care given to patients. Provides supervisory visits as necessary. Willingly accepts direction from the Director of Professional Services, patient care coordinator, or rehabilitation supervisor.
