PHYSICAL THERAPIST - PRN

South Bend Orthopedics is seeking a PRN Physical Therapist for our Unity Hospital location. This position is for out Unity Hospital location. Unity is an elective ortho surgical hospital.This position will be responsible for providing patient care including assessing, interpreting, planning, and implementing special physical therapy treatments.Responsibilities and Duties:Assess patient condition, establish goals, treatments and care plan in conjunction with patientEnsure appropriate treatment is performed in interdisciplinary fashionDocument findings, treatments, and other patient information for medical recordEducate patient on appropriate care and use of various therapies/exercisesComply with appropriate clinic policiesServe as information/education resource for patients, families, and staff.Other duties as assignedRequirements:BS, MA, MS or DPT in Physical TherapyMust be licensed in the state of Indiana.Job Type: PRNHealthcare setting:OutpatientLicense/Certification:Physical Therapy LicenseCPR CertificationWork Location: In personEqual Opportunity Employer
