Penn State Health Medical Group Location: US:PA:Reading Work Type: Full Time FTE: 1.0 Shift: VariedHours: Monday-Friday 8-4:30, Varied Evenings J OB SUMMARY: The Community Health Advocate (CHA) is a frontline public health worker who performs a wide variety of duties at the paraprofessional level to engage, support, advocate and educate community members in overall health and wellness. The CHA serves as a liaison or link between health and social services and the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. MINIMUM QUALIFICATIONS: High School Diploma or equivalent required.One (1) year of experience in a community related role required.BLS certification required within 6 months of hire.Mental Health First Aid training required within 6 months of hire. KNOWLEDGE, SKILLS, & ABILITIES: Bi-lingualCommunication and interpersonal skillsOrganized, detail oriented and motivatedDependable, consistency and professionalismStrong customer service skillsAbility to work with a team, external and internalAbility to capture and report dataExtensive knowledge of community resourcesDevelop and draft goals to address barriers to healthcareAbility to work with patients to reach goals identifiedAbility to identify environments potentially requiring notification to authorities (Child Abuse and Elderly)Reliable transportation and valid PA drivers licenseAble to communicate information in an organized and clear mannerBasic computer skillsMaintains appropriate personal appearanceMeet deadlines PREFERRED QUALIFICATIONS: Community Health Worker Certification preferred.This job description is a general outline of duties performed and is not to be misconstrued as encompassing all duties performed within the position.
