TANF Case Manager (CM)
POSITION SUMMARY:The Case Manager (CM) provides outreach, initial assessment, advocacy, case planning, service coordination, and case management for families referred to the TANF CM program with significant barriers to employment.JOB EXPECTATIONS:Follows all TANF CM policies and proceduresUses a client-centered, strength-based approach to case management servicesProvides culturally and linguistically appropriate case management services to familiesRepresents TANF CM program professionally within the program and in the communityMaintains a positive and respectful work attitudeCommunicates proactively with TANF CM Regional Coordinator and other regional CMsUtilizes the Efforts to Outcomes (ETO) web-based data management system to document all case work in TANF CM programActively participates as a member of the regional TANF CM teamCommits to professional growth and developmentPossesses a valid driver' license and has access to a personal vehicle for regional and statewide travel SPECIFIC DUTIES and RESPONSIBILITIES: Conducts targeted outreach to locate assigned TANF CM referred families within designated program time framesEngages located families in TANF CM services in alignment with expected program outcome standardsProvides intensive, client-centered, in-home and/or community-based case managementPerforms initial client assessments and updated assessments within designated program time framesWorks with clients to develop and implement clear and relevant goals using the Family Development Plan designed to assist clients in overcoming barriers to employmentEstablishes and maintains relationships with local/regional staff from state departments, workforce development boards and others who make referrals to the programTeaches clients budgeting, money management, shopping, banking, and other necessary life skills through direct instruction and/or modelingAssists clients in forming and/or fulfilling an employment plan either through the Jobs First services or on an individual basis for non-Jobs First eligible clientsAssists clients in problem solvingReviews client assessments with TANF CM Regional Coordinator to determine level of serviceRefers clients to appropriate community services based on identified barriers to employment and other family needsWorks with the Regional Coordinator to assess needs for any emergency situations, e.g. domestic violence, child abuse, or acute behavioral health needsAdvocates for clients to maintain or re-establish state benefits and for clients to access needed servicesCommunicates regularly with the DSS/DOL case worker, and provides them periodic updates and closing status reports as appropriateAccurately documents case work daily utilizing Efforts to Outcomes (ETO)Performs other reasonable duties as requested by TANF CM Regional Coordinator and/or TANF CM Agency SupervisorPQI FUNCTIONS:Ensure the consistent accurate collection of the dataIdentify patterns and trends for programUse results of data to inform supervisor of trends that may impact services QUALIFICATIONS:BA/BS in a human service field with two (2) years relevant experience (preferred), Associates or equivalent with over five (5) years of directly relevant experience may be considered in lieu of BA/BS.KNOWLEDGE, SKILLS, and ABILITIES:Thorough knowledge of TFA/TANF eligibility criteria and servicesSignificant knowledge of regional community-based services for low and no income familiesDemonstrated ability to work with culturally and linguistically diverse familiesDemonstrated ability in client-centered, home-based case management servicesDemonstrated ability in teaching basic life and employability skills to adultsDemonstrated ability in creating and implementing individualized service plansAbility to work well independentlyAbility to maintain a flexible schedule, to include non-traditional hours (nights/weekends)Dedicated to promoting cultural wellness on the jobDemonstrated basic computer literacy, ability to work in a Windows environment, and utilization of mobile devicesDemonstrated ability to work in a team approachEMPLOYEE BENEFITSCatholic Charities offers access to a comprehensive array of benefits, including:For Good Health: Medical, Prescription, Dental and Vision insuranceFor a Secure Future: Life, AD&D and Long-term disability insuranceFor Retirement: 403(b) Plan, with employer matching contributions for eligible employeesFor Career Advancement: Training and development opportunitiesFor Work Life Balance: Generous paid time off; including 14 paid holidays, vacation and sick time.Employee Assistance Program (EAP) - Free and confidential counseling for employee and immediate family
