We encourage ideas, innovation, and shared problem‐solving.Position SummaryThe Patient Resource Specialist plays a key role in supporting patients who face barriers to care, especially those at greater risk for health disparities. This position helps patients access social, financial, and supportive services, coordinates care with internal and external partners, and works closely with patients to improve their overall health outcomes. The Patient Resource Specialist serves as a trusted ally, helping patients navigate complex systems and connecting them with the resources they need for a healthier quality of life.This position is ideal for someone passionate about community health, patient advocacy, public health, and social services. We serve a diverse patient population, and bilingual Spanish/English skills are a strong plus for this role.Key ResponsibilitiesIdentify patients in need and assist them in navigating health coverage and supportive services.Serve as a knowledgeable guide to help patients access healthcare, social services, and financial resources.Act as a point of contact to coordinate community‐based services, including:Scheduling appointments with culturally competent providersArranging language translation or interpretation servicesCoordinating transportationConnecting patients and families with support systemsProviding emotional support and relevant informationAssisting with financial support applications and paperworkParticipate in community outreach and education activities.Support local initiatives aimed at improving health outcomes and preventing chronic disease.Collect and document program data for quality improvement, grant reporting, and research.Perform other duties as assigned.Desired Skills & QualificationsExperience in healthcare, human services, public health, or a related field.Strong personal integrity, professionalism, and self‐confidence.Excellent written and verbal communication skills.Ability to empathize, build rapport, and connect with patients from diverse backgrounds.Experience with electronic health records preferred.Bilingual (Spanish) strongly preferred.Demonstrated teamwork skills and ability to collaborate within a multidisciplinary healthcare team.Commitment to the mission and values of Klamath Health Partnership.BenefitsKlamath Health Partnership offers a comprehensive benefits package designed to support your health, wellbeing, and financial stability, including:Medical, dental, and vision coverageEmployer‐paid life and disability insuranceGenerous paid time off and paid holidaysRetirement plan with employer contributionEmployee Assistance ProgramProfessional development and continuing education supportMission‐driven work that directly impacts community healthPhysical RequirementsThis position requires regular walking, talking, bending, lifting, carrying, writing, and hearing.
