Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).Key Responsibilities:The Reentry Care Manager is responsible for leading care coordination activities for enrollees within a Managed Care setting, with specialized duties focused on Reentry Targeted Case Management (rTCM) for individuals transitioning from carceral facilities into the community in Washington state.The primary goal is to ensure integrated, person-centered care that improves health outcomes, reduces recidivism, and ensures seamless continuity of services.Direct experience with the population highly preferred.The Reentry Care Manager (RCM) provides Reentry Targeted Case Management (rTCM) services to justice involved members before and after release from incarceration.This role focuses on whole person, member centered care, ensuring continuity of medical, behavioral health, substance use disorder (SUD), and health related social needs (HRSN) services to support a safe and successful transition back into the community.The Reentry Care Manager develops, assesses, and facilitates complex care management activities for primarily mental and behavioral health needs members to provide high quality, cost-effective healthcare outcomes including personalized care plans and education for members and their families related to mental health and substance use disorder.Key Requirements and Technology Experience:Must have skills: - " LCSW/LMSW/LMFT/LMHC/LPC ", "Re-Entry Case management", “Behavioral health ", "Substance Abuse", “incarceration”Master's degree in Behavioral Health or Social Work.2-4 years of related experience working directly with patients/members.
