A company is looking for a Behavioral Health Care Navigator. Key Responsibilities Support individuals with mental health conditions and substance use disorders through virtual outreach and engagement Screen for unmet behavioral health, social service, and physical health needs, and provide navigation to appropriate care Utilize motivational interviewing techniques to understand and address individuals' care goals and barriers to care Required Qualifications 2+ years of experience in a patient-facing healthcare setting, with a preference for Community Health Worker or licensed social work certification 1-2 years of patient outreach and engagement experience 2+ years of experience in a metric-driven environment Access to a quiet, HIPAA-compliant space for clinical duties Demonstrated experience working with individuals facing complex chronic needs or mental health conditions
