Home Visiting Data Specialist
Position Summary
The Home Visiting Data Specialist is responsible for maintaining the accuracy, completeness, and timeliness of data collection, reporting, and performance tracking for the Luminous Mind Foundation Home Visiting Program. This role supports program compliance, quality improvement, outcome measurement, and funding requirements by ensuring required documentation, assessments, enrollment data, service frequency, and performance indicators are properly captured and reported.
The Data Specialist collaborates closely with Home Visitors, Supervisors, and Leadership to strengthen data integrity, improve workflows, and support program success through effective reporting and monitoring.
Essential Duties and Responsibilities
Data Management & Data Integrity
Collects, reviews, and maintains program data related to enrollments, visits, assessments, screenings, and outcomes.
Ensures timely and accurate data entry into all required platforms and internal tracking systems.
Conducts routine quality checks to ensure data is complete, consistent, and compliant with program standards.
Identifies missing documentation and follows up with staff to correct gaps promptly.
Assessment & Outcome Tracking
Tracks and monitors completion of required Home Visiting assessments, screenings, and tools such as:
ASQ-3 / ASQ:SE-2
EPDS
PICCOLO
Family needs assessments and service plans (as applicable)
Maintains performance dashboards for staff compliance with required timelines and visit frequency standards.
Supports staff with reminders and tracking systems to ensure assessments are completed on schedule.
Reporting & Performance Monitoring
Develops and maintains monthly, quarterly, and annual reports as required by leadership and funders.
Monitors key program indicators such as:
Referrals received and processed
Enrollments and engagement rates
Visit frequency and completion
Assessment completion rates
Caseload/service trends
Program outcomes and retention
Assists in creating visual dashboards, summary sheets, and productivity reports.
Compliance Support & Quality Improvement
Supports internal chart audits and documentation reviews for compliance and accuracy.
Provides data findings to leadership to support quality improvement initiatives.
Assists with preparing for monitoring visits, funder reporting, and internal/external audits.
Supports development of corrective action plans when trends indicate compliance risks.
Staff Support & Training
Provides technical assistance to Home Visiting staff on documentation standards, tracking expectations, and data procedures.
Assists with training staff on required forms, templates, and data submission workflows.
Communicates data-related updates, deadlines, and reporting timelines to team members.
Administrative & Program Support
Maintains secure electronic record-keeping processes aligned with confidentiality expectations.
Supports the coordination of program deadlines and reporting calendars.
Performs other duties as assigned in support of program operations.
Required Qualifications
Associate or Bachelor’s Degree preferred in:
Public Health, Data Analytics, Health Administration, Social Work, Psychology, Education, Human Services, or related field
(Relevant experience may substitute for education.)
Minimum 1–2 years experience in data entry, program reporting, compliance monitoring, or administrative support in health or human services.
Strong attention to detail and high accuracy in documentation and data reporting.
Proficiency in Microsoft Excel or Google Sheets (filters, formulas, sorting, basic reporting).
Strong written and verbal communication skills.
Ability to manage deadlines, prioritize tasks, and work independently.
Understanding of confidentiality and HIPAA-informed documentation practices.
Ability to pass background checks (as applicable).
Preferred Qualifications
Experience in a Home Visiting program or early childhood/family support services.
Experience with grant-funded programs and outcome reporting.
Familiarity with data dashboards, KPIs, or quality improvement processes.
Strong Excel skills (pivot tables, advanced formulas, charts).
Bilingual (English/Spanish) strongly preferred.
Experience working with electronic health records (EHR), case management platforms, or data portals.

Work Environment & Physical Requirements
Office-based role with frequent computer and phone work.
Prolonged sitting and data entry.
Occasional travel for program support, audits, or meetings (if applicable).
