Hospice RN Case Manager

Job DescriptionPathway Hospice, the leading locally owned hospice provider in DFW, is seeking an experienced RN to join our wonderful team as a Hospice Case Manager. The ideal candidate will be dedicated to providing exceptional care for our patients and their families. While our office is in Richardson, TX, we are currently recruiting for several DFW territories.Pathway offers an excellent benefits package, including but not limited to:Competitive SalariesCompany pays 70% of Employees’ Medical InsurancePaid Time OffCompany CarResponsibilities of this position include the following:Develops, coordinates, and follows the patient's individualized plan of careCase manage total care of patient by coordinating with team members/physicians, supervising CNAs and participating in staff and Interdisciplinary Team meetingsAssess patient’s physical condition according to Plan of Care; determine and/or revise plan of care regarding medications and DMEAppropriately identifies/utilizes resources: Dietary, Pharmacy, Supplies, Home Medical Equipment, etc. Serves as a patient and family advocate; Maintains focus on the patient and family experience of careCompletes documentation in a timely mannerProvides education to patients, families, and caregiversParticipates in patient care meetings as assignedCommunicated effectively with coworkers and patients/familyParticipates in bereavement supportJob Type: Full-timeCompany DescriptionWhile all hospices are required to offer the same primary services, each agency has a distinctly individual approach to service delivery.
