Core Job Responsibilities Assessment: Performs and documents assessments and reassessments in compliance with policies, demonstrating the ability to make independent decisions based on patient condition. Plan of Care (Development & Implementation): Initiates and prioritizes patient care plans, collaborating with patients, families, and interdisciplinary teams to meet needs. Evaluation: Monitors and documents the patient's response to clinical interventions. Education: Provides, coordinates, and evaluates education for patients and families tailored to their needs during the stay and for discharge. Leadership & Teamwork: Delegates tasks to licensed and non-licensed personnel, acts as a preceptor/mentor, and fosters teamwork. Quality & Improvement: Identifies opportunities for improvement and participates in quality initiatives.
