Mental Health Professional, Program Evening Coordinator

Support patients and psychotherapists in the implementation and continued adherence to each patient's treatment plan. Patients typically are suffering from alcohol and opioid abuse stemming from conditions such as Schizophrenia and Major Depression. Pilsen Wellness Center (“PWC”) in most instances will rehabilitate a patient with a dual approach, dealing with both the mental health and substance abuse disorders at the same time. Treatment usually includes medication training and monitoring; individual and group therapy sessions; providing education on symptom management, coping skills, home management and independent living skills in order to foster independence and reintegration back into the community. In some instances, PWC will prescribe FDA approved treatments to mitigate the effects of withdrawal. In this context, the Mental Health Professional, Program Evening Coordinator, will have the following main duties: (1) plan and evaluate patients' behavior as between individuals and within the group. (2) Guide and counsel patients in their on-site residential maintenance including, but not limited to, bed making, cleaning bedrooms, personal hygiene, and washing dishes. (3) Provide assistance to patients in crisis and emergency situations. (4) Participate in weekly team meetings to coordinate patient care with psychotherapists and other personnel. (5) Conduct building rounds to ensure patients' safety and security. (6) Participate in monthly online training with the State of Illinois. (7) Document each patient's progress within the Community Innovation Service software system including tracking important events, medication changes, treatments, and patients' symptomology. (8) Comply with State reporting requirements including updating patients' IM-CANS profile with the State of Illinois and filing periodic reports under Illinois Medical Rule 132. Complete statistical form 1077 with the Illinois Department of Human Services.
