Wound Kinetics – Nurse Practitioner (NP) / Physician Assistant (PA) JobDescription (Chicago, Illinois)Position Title:Nurse Practitioner (APRN) or Physician Assistant (PA) – Mobile Wound Care SpecialistLocation:Chicago, Illinois (SNF, LTC, ALF, Home Health settings – mobile model)Position SummaryThe NP/PA will provide comprehensive wound care services across skilled nursingfacilities, long-term care, assisted living, and home-based settings. This role focuses ontraditional and advanced wound management, bedside procedures, andinterdisciplinary collaboration to improve healing outcomes and reducehospitalizations.Core ResponsibilitiesClinical Care Perform comprehensive wound assessments (pressure injuries, diabeticulcers, venous/arterial wounds, surgical wounds) Develop and implement evidence-based treatment plans Perform bedside procedures including:o Sharp/mechanical debridement (within credentialing and training)o Incision & drainage (when appropriate)o Skin substitute applicationo Management of Negative pressure wound therapy management Order:o Labs, cultures, imagingo Vascular studies (ABI, duplex ultrasound where applicable) Prescribe medications and treatments in accordance with Illinois regulationsFacility-Based Care Model Conduct weekly rounding in assigned facilities Collaborate with:o Directors of Nursing (DONs)o Facility administratorso Therapy teams Provide real-time care plan updates and documentationDocumentation & Compliance Complete timely documentation aligned with:o Medicare LCD guidelineso ICD-10 / CPT coding requirements Ensure compliance with:o Facility policieso State and federal regulations Participate in audits and quality assurance programsCare Coordination & Education Educate nursing staff on:o Wound prevention protocolso Dressing techniqueso Offloading and positioning Communicate with:o Patients Familieso Primary care providerso Other SpecialistsOperational & Growth Responsibilities Participate in:o Monthly facility reviewso Outcome tracking (healing rates, new wounds, hospitalizations) Support business development:o Relationship building with facilitieso Clinical program differentiation Assist with implementation of new service lines:o Advanced therapiesScope of Practice Considerations (Illinois)For Nurse Practitioners (APRN) Must maintain:o Illinois APRN licensureo National certification Collaborative agreement required unless full practice authority criteria met May:o Diagnose, treat, and prescribe within scopeo Perform procedures based on training and privilegingFor Physician Assistants Practice in collaboration with a physician who is available for consultation Scope defined by:o Supervising physiciano Experience and competencies Perform procedures and prescribe per delegation and credentialingQualificationsRequired NP (FNP, AGNP, or Acute Care) or PA-C Active Illinois license DEA license (or ability to obtain) Minimum 1 year’s clinical experiencePreferred Wound care experience (SNF, LTACH, outpatient wound center) Certification:o WCC (Wound Care Certified)o CWCN / WOCN Preferred Experience with:o Debridemento Advanced biologics / skin substituteso Mobile or facility-based careCompensation Structure (Aligned with Wound Kinetics Model) Daily Base Salary + productivity model Productivity BonusWork Environment Mobile role (travel between facilities) Flexible scheduling based on rounding needs High-autonomy clinical decision-making environment
