Location: Jersey City, New Jersey
Schedule: Full-Time | Monday–Friday | 9:00 AM – 5:00 PM
Salary: Up to $90,000 annually (based on experience)
Join Our Team as Director of Social Services
We are seeking an experienced and compassionate Director of Social Services to lead and oversee the Social Services Department within our skilled nursing facility. This leadership role is ideal for a dedicated professional with strong regulatory knowledge, nursing home experience, and a passion for resident advocacy.
Key Responsibilities
Develop, implement, and maintain Social Services policies and procedures in compliance with federal, state, and facility regulations
Plan, organize, and supervise all administrative and operational functions of the Social Services Department
Recruit, supervise, and retain qualified Social Services staff
Coordinate staff training, in-services, and continuing education
Participate in interdisciplinary care plan meetings and facility-wide committees
Oversee the admissions process to ensure a smooth and supportive transition for new residents
Serve as Discharge Planning Coordinator, identifying and resolving barriers to safe and timely discharges
Act as the Grievance Officer, addressing resident and family concerns promptly and compassionately
Promote and enforce the Resident Bill of Rights
Collaborate with hospitals, healthcare providers, and community agencies to ensure continuity of care
Monitor and evaluate the quality and effectiveness of social services
Ensure compliance with survey readiness, audits, and regulatory documentation
Prepare and submit monthly, quarterly, and annual reports as required
Conduct internal audits and quality assurance reviews related to Social Services
Requirements
Bachelor’s degree in Social Work (BSW) required
Master’s degree in Social Work (MSW) preferred
Registered with the Academy of Certified Social Workers (ACSW) preferred
Social Worker Certification (Required)
Minimum of 2 years of experience in a nursing home or skilled nursing facility – REQUIRED
Must be knowledgeable of federal, state, and local regulations governing nursing care facilities
Ability to make independent decisions and exercise sound judgment
Strong leadership skills with the ability to work harmoniously with and supervise staff
