Job Description Physicians Choice Home Health is hiring a full-time or part-time/per diem Physical Therapist (PT) to join our Home Health team, serving patients in the South Bay of Los Angeles. In this role, you will provide high-quality physical therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will oversee a supporting Physical Therapy Assistant and a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care. Before applying for this role, please read the following information about this opportunity found below.What You'll DoProvide in-home physical therapy evaluations and treatmentsAssess patients' strength, mobility, balance, and ability to function safely at homeCreate and update individualized plans of care that support independence and recoveryHelp patients regain mobility and confidence in their home environmentEducate patients and caregivers on exercises, safety, and fall preventionDocument visits and evaluations in WellSky/Kinnser in a timely mannerCollaborate with nurses, therapists, and the broader care team to support patient outcomesSupervise Physical Therapy Assistants (PTAs) and support their clinical developmentParticipate in care planning, case conferences, and discharge planningRequirements Graduate of an accredited Physical Therapy program (Bachelor's, Master's, or DPT) Active California Physical Therapist license CPR/BLS certification Strong communication and organizational skills Reliable transportation and willingness to travel to patients' homes Comfort working independently in a home health setting Home health experience preferred Experience charting in Kinnser/Wellsky preferred, but not requiredBenefits$40-110/hour depending on visit/task typeMileage reimbursement Cellular device reimbursement Eligibility for Obran Cooperative membership and profit-sharingFull-time packages also includeMedical, dental, vision, life & disability insurance, with generous employer contributionsHSA/FSA options to cover healthcare expenses and dependent care with pre-tax dollars401(k)Paid time off, sick time, and paid holidaysSalary Range Disclaimer: The rates listed represent the low and high end of the range for this position.
