Travel Physical Therapist - Home Health

American Traveler is seeking a licensed Physical Therapist with skilled home health experience for a 13-week home health assignment in the Hood River, OR area.Job Details• Home health setting serving Hood River, OR and surrounding areas,• 1:1 patient ratio with up to 6 patient visits per day; standard productivity of 30 points per day,• Day shift schedule, 5x8-hour shifts (8:00 AM - 4:30 PM); 4x10 and 3x12 schedules are not available,• No weekend or on-call requirements,• Charting via Epic Remote Client (laptop) and Rover app (mobile); OASIS documentation required,• Travelers may be required to float between facilities within the state of Oregon,• Travel between patient locations required; occasional maximum driving radius of approximately 60 miles (~60-90 minute total drive time per day),Job Requirements• Minimum 1 year of skilled home health PT experience required,• Active Oregon PT license required,• BLS certification required,• Epic EMR experience required,• OASIS experience required,• Must have reliable personal transportation,• COVID-19 primary vaccination series plus at least one booster required,• License required if applicable,• Minimum of 2 supervisory references covering 1 year of employment within the last 3 years in a comparable specialty or setting required for consideration,• SSN and DOB required for consideration,Additional Information• Responsibilities include home-based PT evaluation and treatment, with patient visits grouped geographically to minimize drive time,• Business casual attire required; scrubs are permitted but no specific color required; jeans are not allowed,• First-time travelers are welcome to apply,• Local candidates are accepted; candidates residing within a 50-75 mile radius of the facility are considered local and will be accepted at the same rate,• Former permanent staff must wait at least 3 months before returning as a traveler and must not reside within 50 miles of the submitting facility,• Travelers who refuse to float between facilities within the state may be subject to assignment termination,
