Infusion Pharmacist

The Specialty Pharmacy Infusion Pharmacist plays a central clinical and leadership role in ensuring safe, compassionate, and coordinated infusion services in the home and ambulatory infusion suite settings. This pharmacist serves not only as a clinical expert, but also as a trusted healthcare partner to patients, caregivers, providers, and community organizations. The role blends advanced clinical oversight with accessible, patient-first service that reflects the values of independent, local ownership. In a locally owned infusion pharmacy, the pharmacist is not just a verifier of orders — they are a cornerstone of the organization’s clinical integrity and community reputation. This role directly influences patient outcomes, provider trust, and the long-term success of an independent healthcare model. Patient-Centered Clinical Management Conduct initial and ongoing clinical assessments for patients receiving home or ambulatory infusion therapies. Develop individualized care plans based on diagnosis, lab values, comorbidities, and patient-specific factors. Provide comprehensive counseling to patients and caregivers on: Medication administration techniques (IV push, elastomeric devices, ambulatory pumps) Storage and handling Potential side effects and infusion reactions Emergency response procedures Monitor laboratory parameters, therapeutic response, and adverse events. Communicate proactively with prescribers regarding therapy adjustments and clinical recommendations. Support safe transitions of care from hospital to home or infusion suite. Infusion Therapy Oversight Review and verify infusion orders for appropriateness, dosing accuracy, stability, and compatibility. Oversee compounding in compliance with USP (for sterile preparations) Ensure proper beyond-use dating, storage, and cold-chain integrity for shipped medications. Collaborate with nursing staff to manage infusion-related reactions and troubleshooting. Provide clinical oversight for biologics, immunoglobulins, anti-infectives, TPN, inotropes, and other specialty infusion therapies. Home Infusion Coordination Coordinate medication shipment logistics and delivery schedules to align with nursing visits. Ensure proper patient training prior to initiation of therapy. Provide telephonic clinical support for patients and home health nurses. Evaluate patients for appropriateness of home infusion vs. alternate sites of care. Assess patient safety factors (home environment, caregiver support, infection risk). Ambulatory Infusion Suite Support Collaborate with infusion suite nurses to optimize workflow and patient scheduling. Provide chairside or consult-based patient counseling as needed. Review pre-infusion labs and therapy parameters before treatment. Assist in management of infusion reactions and emergency protocols. Support clinical protocol development for chronic infusion therapies. Quality, Compliance & Growth Maintain compliance with all state and federal regulations and accreditation standards (ACHC). Contribute to quality improvement initiatives and clinical outcome tracking. Support development of new infusion service lines and specialty programs as the organization grows. Assist ownership in maintaining high clinical standards while supporting sustainable business operations. Required Doctor of Pharmacy (PharmD) degree. Active pharmacist license in good standing. Experience in infusion pharmacy, specialty pharmacy, hospital, or ambulatory care setting. Knowledge of sterile compounding regulations and infusion therapies. Preferred Experience in home infusion operations. Familiarity with infusion pumps and home infusion delivery systems.
