St. Mary’s Healthcare has also been named a 2026 Times Union Top Workplace—the first acute care hospital in the Capital Region to earn the distinction since the Times Union launched the recognition program 14 years ago.Job Requisition: SOCIA001578Employment Type: Full-timeShift: DaysHours Per Week: 40-hour/weekHiring range: $25.00-$32.00 hourlyThe actual compensation for this position will be determined based on experience and other factors permitted by law.ResponsibilitiesMaintaining and/or improving resident’s ability to control everyday physical needs and mental/psychosocial needsInitiating appropriate interventions for residents experiencing mental or behavioral health issuesIdentifying individual social and emotional needsParticipate in the planning of the resident’s admission, community re-entry, or transfer to another facilityWork with the finance team to ensure the resident is educated and the proper documents are in place for the resident during his or her stayWork with the resident on discharge and get necessary DME or home care in place so the resident is safe when he or she returns to the communityOther duties as required for the safety and care of the residentsQualificationsNursing Home or Hospital experience. Internship is sufficient. (Required)BSW, MSW or BA/BS in related field. (Required)Bi-lingual English/Spanish (preferred but not required)EducationBachelor's Degree required Minimum years of experience - 1 yearRequired Skills & AttributesAbility to organize and establish day-to-day priorities while utilizing critical thinking skills in all aspects of the job. Must be able to multitask while remaining professional, focused, composed and positive. Excellent customer service skills and must display integrity, friendliness, and compassion.
