Physical Therapist Case Manager/Kids Path - PRN

Our Expert Care.AuthoraCare Collective is currently seeking a PRN Physical Therapist Case Manager/PT Kids Path for our Home Health department supporting the following counties: Alamance, Orange, Durham, Person, Chatham, Caswell, Rockingham, Guilford, Stokes, Forsyth, Davidson.The Physical Therapist provides services to retore function, improve mobility, relieve pain, prevent, or limit permanent physical disabilities of patients suffering from injuries and disease. Manage the therapy Plan of Treatment for a case load of Integrated Health patients in collaboration with other members of the interdisciplinary team (IDT). Assess and identify patient problems, establish therapy goals, and interventions.Work with physician to establish a treatment plan using a variety of methods, including exercises, stretching maneuvers, hands-on therapy, and equipment to ease pain and help increase mobility and endurance. Establish and maintain therapeutic relationship with patient and/or caregiver, including providing community support for resources and education. Document the goals and outcomes of care.Therapist may perform evaluations and treatments for hospice and Kids Path Home Health patients as needed. The Therapist will bring experience and passion in working in an environment that follows AuthoraCare core values of humanity, compassion, empowerment, and excellence as well as inclusion of all team members, patients, and caregivers.Education and Experience:Graduate degree from an accredited Physical Therapy Program (approved by the APTA)Minimum of one-year physical therapy experience home health or facility preferredCurrent and unrestricted Physical Therapy LicensureCurrent CPR certificationPatient management skills needed include excellent patient assessment, critical thinking, problem solving, delegation, effective interpersonal and communication skills necessary.Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
