Job DescriptionPrimary Care Geriatrics PhysicianAcclaim Multi-Specialty Group | Fort Worth, TexasThis position focuses on outpatient primary care with a geriatrics patient population.Outpatient clinic settingPrimary Care Geriatrics patient panelEssential Duties and ResponsibilitiesCoordinate multidisciplinary care for assigned patient panels to optimize outcomesReview patient medical records to diagnose, treat, and provide care management and education to patients and familiesMake appropriate patient referrals as neededChampion a culture of empathy, dignity, and respect for aging populationsMonitor national health guidelines and those established by JPS Health Network and Acclaim Multi-Specialty Group for patient care managementProvide evidence-based, compassionate care to improve patient outcomes and increase patient satisfaction scores while demonstrating patient-centered behaviorsMaintain a collaborative environment with referring physicians, residents, medical students, advanced practice professionals, and support staff to ensure quality patient careSupport academic, educational, and research endeavorsAssess and manage chronic and acute conditions in patients with multiple comorbiditiesPartner and communicate effectively to support a positive brand and patient/customer experience for JPS Health Network and Acclaim Multi-Specialty GroupServe as a resource to the communityPerform other job-related duties as assignedQualificationsRequired Education and ExperienceDoctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited school of medicineResidency and/or fellowship training in Family Medicine (Geriatrics)Required Licensure and CertificationCurrent, unrestricted medical license to practice in the State of TexasBoard Certified or Board EligibleUnrestricted authorization to work in the United StatesPreferred QualificationsBilingual or multilingual proficiencyPrior experience in post-acute or long-term care settingsProficiency with electronic medical records and knowledge of CMS/SNF regulatory requirementsKey CompetenciesAbility to work comfortably in a large public/county healthcare facilityDemonstrated knowledge and competency in assessment techniques, diagnosis, and treatment processes within the assigned specialty areaStrong interpersonal, verbal, and communication skills to collaborate with healthcare professionals and serve as a community resourceSelf-directed with the initiative to participate in community activities and medical outreach programs related to the specialty areaOpportunity to supervise Advanced Practice ProfessionalsBenefitsCompetitive salaryGenerous ATO timeMalpractice insurance provided at no costHealth, vision, and dental insurance optionsProfessional expense allowance
