The OT will evaluate patients, develop treatment plans, and deliver therapy focused on improving functional independence and daily living skills.Must-Have Skills: Active Illinois Occupational Therapist License Strong patient assessment and treatment planning skills Experience in outpatient rehabilitation settings Knowledge of ADL training and therapeutic interventions Excellent communication and teamwork skillsExperience Required: Minimum 3 year of OT experience required Outpatient rehab experience preferredKey Responsibilities:• Evaluate patients and develop individualized treatment plans• Provide therapy focused on functional independence and ADLs• Monitor patient progress and modify treatment plans as needed• Educate patients and caregivers on therapy techniques• Maintain accurate and timely documentation in EMR• Collaborate with interdisciplinary care team• Ensure compliance with facility policies and proceduresCloudinfraIT Inc Job ID #296048.
