Minimum Qualifications Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA. Active KY license or compact license required to start the assignment. Demonstrated home health experience. Experience with HCHB electronic medical record and OASIS documentation. Ability to meet productivity requirement of 30 points weekly. BLS certification from AHA or ARC may be required. Experience performing comprehensive PT evaluations, establishing measurable goals, developing and modifying treatment plans, and documenting per company, facility, and regulatory requirements. Ability to supervise PT Assistants and PT Aides in accordance with state practice act. Preferred Qualifications Prior experience working in home health environments covering multiple counties such as Powell, Menifee, and Montgomery. Familiarity securing and incorporating adaptive equipment into patient programs. Assignment Details Shift: Day shift, 8-hour days. Work schedule: 5x8. Counties covered: Powell, Menifee, and Montgomery. On-call schedule: none. Must be willing to travel within the local county coverage area as needed. Impacting Patient Care Perform physical therapy evaluations and treatments based on physician orders and referrals. Complete comprehensive evaluations using appropriate assessment tools. Establish measurable goals and develop treatment plans to advance patient independence and function. Reassess treatment effectiveness and communicate patient needs and progress to the care team, physicians, patients, and families. Recommend and assist in securing adaptive equipment. Maintain evaluation, treatment, and billing documentation in required formats and timelines. Adhere to HIPAA, corporate compliance, professional practice standards, and applicable state and federal laws. Participate in staff development and maintain required continuing education.
