Job description Reviving Mind is a rapidly growing digital health company integrating Chronic Care Management, Remote Patient Monitoring, and Collaborative Care for Senior Care Facilities. We are at the forefront of integrative and preventive medicine. We are seeking an experienced Advanced Practice Registered Nurse Practitioner (APRN) in Illinois: Reviving Mind: -Conducts new patient visits to develop a Chronic Care Management Plan: -Problem list, specific measurable goals for specific chronic problems -Patient-populated personal goals -Other providers, community resources - Care gaps identified and addressed - Symptom and medication management addressed -Determines and documents health histories -Provides medical advice and information to client during each initial visit to ensure understanding by the client of his/her condition and the Chronic Care Management services provided and goal of Reviving Mind Lifestyle Medicine Program - Supervise and sign off on subsequent visits of clients with lifestyle medicine coaches that conduct vion-site visits to improve chronic care management by focusing on pillars of sleep, nutrition, physical activity, mindfulness, substance control( alcohol, tobacco), and social connectedness. -Reviews and supervises coaches, makes effective suggestions and recommendations that are given to clients in the Lifestyle Medicine program for the goal of chronic care management -Consults with other health care team members in regards to client treatment -Prepares medical record of client encounters -Refers client to outside medical and support facilities -Maintains an on-going health education program with the assistance and guidance Chief Medical Officer Knowledge, Skills and Abilities: Knowledge of evaluation plans and treatment of clients with a variety of chronic illnesses including obesity, hypertension, diabetes, smoking/alcohol cessation Knowledge of lifestyle medicine and health coaching is preferred but not required Knowledge of supervisory practices and principles. Skill in patient care. Skill in both verbal and written communication. Skill in the use of medical equipment utilized in area of assignment. Minimum Qualifications: Bachelor's degree in Nursing from an accredited Nursing program AND satisfactory completion of an accredited Nurse Practitioner program AND three years of professional Nurse Practitioner experience. Current licensure to practice nursing in Illinois as a Nurse Practitioner by the State Board of Nursing Please send cover letter and resume for consideration.
