Social Services Director

Healthcare Facility - Our Skilled Nursing Facility, located near Crete, IL is seeking a Social Services Director who is self-motivated, has previous experience, is organized, and has excellent communication skills. The Social Services Director will assist us in achieving our mission of growing a quality-oriented and employee-friendly organization.Social Services Director Duties and Responsibilities:Develop ongoing care plans based on residents' needs, interests, and abilities identified in initial assessmentsParticipate in care plan meetings to ensure a comprehensive and coordinated approachComplete required documentation upon resident admission, discharge, and transferImplement healthcare directives and identify end-of-life care opportunitiesAssess residents' potential for discharge in collaboration with the interdisciplinary teamPlan and coordinate discharge services with residents, families, and community resourcesAssist residents and families in navigating the discharge process and ensuring a smooth transitionSocial Services Director Qualifications:Bachelor's degree in Social Work (BSW) or related field2-3 years of experience in social services/discharge planning within a long-term care or skilled nursing facilityStrong working knowledge of Medicare, Medicaid, and insurance processesExcellent communication and leadership skillsAbility to work as part of an interdisciplinary teamBenefits:We offer a competitive salary and excellent benefits package.
