The Dementia Program Coordinator is responsible for providing overall direction of the Memory Support Neighborhood. Responsibilities include promoting excellence in resident care in accordance with each resident’s Individualized Service Plan (ISP), programming, providing outreach and support to families, being resourceful and knowledgeable with the evolving information on Alzheimer’s disease. Responsible for championing the Covenant Woods’ approach to delivering care to those with dementia, assisting in the training of team members and other stakeholders.Some Job Specifications:Plans, organizes, develops, and leads the overall programing of the Memory Support Neighborhood in accordance with federal, state and local laws, and Covenant Woods’ standards, guidelines, and regulations.Designs, schedules, and facilitates the dementia program incorporating individual resident function and preference while offering physical, mental, and spiritual activities.Partners with the AL Manager to ensure a variety of appropriate and individualized activities are available throughout the day and evening and that direct care staff are actively involved and engaged with activities.Partners with the AL Manager to ensure that the residents clinical needs are addressed.Participates in tours and the evaluation process of prospective residents.Reviews, prior to move-in, the resident’s preferences and needs and schedules the Family Meeting to discuss the Resident Profile.Facilitates the Individualized Service Plan (ISP) and educates staff to ensure service delivery, safety, hydration, and resident engagement while living in the Memory Support Neighborhood.Manages the assessment process to determine levels of care and works with AL Manager for appropriate placement.Reviews daily documentation pertinent to a resident’s physical and behavioral changes.Understands resident changes in condition and takes appropriate action including communication to family and physician. Document on resident’s charges are required by regulation, department and organization guidelinesOversees planning for family events and services.Knowledge and Skill:To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required.College degree preferred.Extensive knowledge/continued education in dementia. Must be able to coach others in dementia programing.One (1) year supervisory experience preferred, including coaching, and daily operations supervision.Ability to handle multiple priorities.Ability to delegate assignments to the appropriate individuals based on their skills, roles, and interests.Competent in organizational and time management skills.Demonstrates good judgment, problem solving, and decision-making skills.Demonstration of proficiency in computer skills, Microsoft 360 (Windows, Outlook, Excel, Teams), and the ability to learn new applications and EMR software.Ability to work weekends, evenings, and flexible hours to be available to meet the needs of the organization.Covenant Woods understands that having a passionate staff is at the center of what makes us great.
