A Home Care Visiting RN provides safe, holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self-management and provides support to meet the needs of the patient, family, and community.Ability to perform Essential Functions as well as physical requirements of Position.Competent to safely and independently perform and/or provide patient education where applicable to/on the following:Wound Care, including Wound V.A.C.Hand HygieneBlood Glucose TestingBasic LiftMechanical LiftMedication ReconciliationExperience/Familiarity with Electronic Medical RecordWorking knowledge of home care regulation, eligibility, and best practicesPREFERRED, BUT NOT REQUIRED:Pleurx and Pleural CatheterPleural DrainageTimed Up and Go (T.U.G.) testingRequirement description : Auto insurance, vehicle and valid driver's license required for all qualified candidates!Minimum of 1 year of home care experience - REQUIREDHome Care Home Base and OASIS - REQUIREDCopy of Driver's License and auto insurance included with submission - REQUIREDBLS (AHA or ARC) - REQUIREDCOVID test required before starting assignment UNLESS already fully vaccinatedPrior travel experience - Preferred
