Advanced Psychoeducational Specialist- Clermont Hospital

Primary Function/General Purpose of PositionThe Advanced Psychoeducation Specialist provides direct patient care under the guidance of a designated healthcare professional in accordance with federal, state, and local regulations, and within policies, procedures, and guidelines of Bon Secours Mercy Health (BSMH). May provide clinical and administrative supervision.Essential Job FunctionsFacilitates groups. Provides Individual activities tailored to specific needs of patients (within scope). May participate in family meetings and treatment team meetings. Discharge planning in collaboration with Social Worker team Community resource knowledge and coordination of activities; particularly in the area of specialized activities such as pet therapy, nutrition, yoga. Completes leisure assessments Care planning May provide one-on-one (1:1) patient safety companion service as needed. May provide 15-minute patient checks as assigned. Assists with the onboarding of new and/or entry level Psychoeducational Specialists May take on more complex tasks and/or projects as needed Successfully participates to complete BSMH and departmental specific training as required This document is not an exhaustive list of all responsibilities, skills, duties, requirements, or working conditions associated with the job.
