HOME VISITOR RESPONSIBILITIES:
Evaluating the effectiveness of the plan of care for the individual
Assessing the participant’s health, welfare and unmet needs
Developing back-up plans and goals with the client
Communicating with SeniorsPlus Care Coordinators to assist clients in receiving appropriate, effective and efficient services to achieve the maximum amount of independence possible
Positively impacting and improving the well-being and health care outcomes for disabled children, adults and older people who are participating in at-home, long-term care programs as prescribed.
EDUCATION and/or EXPERIENCE
Bachelor's Degree in Social or Human Services or Health related required
Familiarity with the long-term care system a plus, strong computer and organizational skills required
Valid Driver's License a Must
Job Type: Part-time/Per Diem
Home Visit rate of $50 per visit after training at hourly rate (from $22/hour)
