Job Title: Wellness Coach / Case ManagerLocation: Brooklyn, NYPay Rate: $22.00 – $24.00/hour (Contract)Schedule: Sunday - Tuesday and Thursday (10am-6pm) and Wednesday 2pm-10pm. (Fridays and Saturdays Off)About the RolePride Health is seeking a Wellness Coach / Case Manager to support individuals in the Bronx, NY. This role focuses on helping consumers access vital community services such as medical, psychological, vocational, housing, and daily living support.
