ESSENTIAL DUTIES AND RESPONSIBILITIES: ➢ Administer the pre-employment tests (Drug screening/TB) as well as oversee and complete the in-service TB Testing and random drug screening for the staff. ➢ Coordinates, organizes, and conducts the Core A and B Training for the staff as per the State of Indiana guidelines. Need to be certified by the State of Indiana, within the first 3 months of the job. ➢ Proper documentation of all the tests and training both the hard copy as well as digital. ➢ Periodically review the Staff data for the validity of the tests and training, follow up, organize, and conduct the tests and training and document it. ➢ Organizes and conducts CPR training for the staff. ➢ Conduct any other health-related training not mentioned herein, as the case takes place. ➢ Completes an initial assessment of the client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es) ➢ Conducts periodical home visits to assess the client’s health problems and needs as well as develop and implement a nursing care plan. ➢ Regularly re-evaluates client’s nursing needs. ➢ Initiates the plan of care and makes necessary revisions as client’s status and needs change. ➢ Communicates with community health related persons to coordinate the care plan. ➢ Monitoring all aspects of patient care, including diet and physical activity. ➢ Maintaining accurate, detailed reports and records and keeping the HOC CRM up to date. ➢ Coordinate/oversee/supervise the work of Direct Support Professionals and Personal Care Workers and provides written personal care instructions/care plan that reflect current plan of care through the CRM. ➢ Monitor the appropriate completion of documentation by the DSPs/personal care workers as part of the supervisory responsibility. ➢ Provide clear and specific instruction for the staff related to the client’s medical diagnosis, allergies, medication, diet, special emergency plans etc. ➢ Prepare appropriate medical documentation for all clients, including any case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. This includes but is not limited to hospital admissions, hospital discharges, medication orders, contacting the physician, medication plans, etc. ➢ Coordinate and complete the High-Risk Plans for the clients. ➢ Completes the quarterly client visit with proper documentation. ➢ Functions as part of an interdisciplinary team attending medication rounds and team meetings for Individualized Support Plan (ISP) and Quarterly Review. ➢ Is available for ongoing consultation with the client homes and team regarding medical problems and health care. ➢ Completes the medical sections of the ISP and Quarterly Reviews that address health and illness needs and issues. ➢ Documents services in accordance with Agency policy and requirements. ➢ Promotes resident empowerment and self-care activities including self-medication. ➢ Responsible for the reporting – e.g., Incident reports (providing data), Mortality Review Report or any other reporting pertaining to the health and safety of the client as prescribed by the State of Indiana. ➢ Maintain up to date stock report of the medical supplies in the Nurse’s office and responsible for the timely replenishment. ➢ Any other additional task as assigned or as it may arise with the changes. ADDITIONAL DUTIES AND RESPONSIBILITIES ➢ Covers for peers during vacations and at other times of Agency need. ➢ Participates in Agency training activities. Teaches and reinforces vital signs and medication management to home staff. ➢ Participates in the orientation of new nursing staff. ➢ Participates in community and other public education regarding mission and philosophy. ➢ Attends staff, discipline and committee meetings as assigned by Supervision. ➢ Reviews and signs incident reports and provides follow-up. ➢ Assumes responsibility for professional development. ➢ All other related duties as assigned by the Management from time to time and/or required by the BDS, Indiana Pathways, or any other governing body as and when there are any amendments or addition of the services. SUPERVISORY RESPONSIBILITIES: None (overseeing the completion of task and wellness and any other requirements to be completed by the DSPs and reporting to the Staff Supervisor or Scheduler) QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE: ➢ Bachelor’s degree in nursing. ➢ Two years of experience working as a registered nurse including some home care/home health care/medical/surgical experience and awareness of the needs of the chronically, mentally ill and developmentally disabled. LANGUAGE SKILLS: ➢ Ability to read, analyze, and interpret human service periodicals, professional journals, technical procedures, or governmental regulations. ➢ Ability to write reports, business correspondence, and procedure manuals. ➢ Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public. MATHEMATICAL SKILLS: ➢ Ability to calculate figures related to medication dosages, and amounts such as discounts, interest, commissions, percentages, and area. REASONING ABILITY: ➢ Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. CERTIFICATES, LICENSES, REGISTRATIONS: ➢ State of Indiana License as a Registered Nurse. ➢ Must have and maintain a valid Indiana Driver’s License, maintain automobile insurance coverage, and have access to an automobile. Will be required to drive daily in Marion and/or surrounding County for meetings and/or visits to residential facilities. Will work in varied residential environments based on assigned caseload.
