Hospice Care PositionNewton & Surrounding AreaEssential Duties and Responsibilities:Ensure quality and safe delivery of hospice services to patients, families, and caregiversPerform or assist patients with personal care activities described in the plan of care, including but not limited to hygiene, mobility and elimination. If not able to perform a task, report to the primary RN case manager immediatelyPerform and maintain a safe environment for patientsUse equipment safely and properlyProvide universal for conscience and all contacts with patientsPerform or assist patient/family with home maintenance activities as described in the care plan, including but not limited to light housekeeping and meal preparationPerform support activities as prescribed in the care plan including but not limited to supporting independence, providing companionship and providing respite for the family/caregiversRespect the autonomy and right to confidentiality of all patients and families/caregiversMonitor adequate fluid and nutrition intake, temperature, pulse, respirations, skin and basic elements of body functions and record/report appropriatelyPractice safe transfers, ambulation and normal range of motion and position techniquesIdentify the need for intervention by other members of the interdisciplinary team on an ongoing basis and report to primary RN case managerReport on the effectiveness of the care planDocument care provided in response with patient and family/caregiver submit documentation in a timely manner per agency policyAssist patient, family/caregiver and other team members and providing continuity of careWork in cooperation with IDG to meet the emotional needs of the patient's family/caregiverManage a caseload and deliver care as prescribed in the care plan use supplies and equipment effectively and efficientlyComplete a minimum of 12 hours of in-service per year to include dementia trainingActively participate in team meetings and contribute positively to the patient's care planningParticipate in QAPI, special projects and perform other duties as assignedDesired Qualifications:High School graduate or GED requiredProof of certification by state of MassachusettsA minimum of one year of direct care experience in an institute of home care settingExperience in hospice is highly desirableAbility to effectively communicate in a professional and composed manner with coworkers, patients and familiesJob Performance Criteria:Sit and stand during most of the workdayShould be able to stand and slash walk for at least half an hour at a timeCommunicate verbally and in writing with the interdisciplinary team patients, families and referral sourcesMust demonstrate skills and written slash pro competency prior to working independently with the patientSufficient documentation for having passed a skill test
