LPN Quality, Data & Patient Outreach - TMG Operations (Days)

This role is an LPN who supports our Quality and Data Analytics team. Primary duties to include entering and preparing AWV-related orders in advance of patient visits, conducting outreach calls to patients, reviewing documentation for completeness for providers, and coordinating with practices for scheduling patients. This role works with all TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner.Required Knowledge & SkillsEducation: Associate Degree or 2 years of college courseworkExperience: One year of related experience. Requires a working knowledge of standard practices and procedures.Licenses and CertificationsGEORGIA LICENSED PRACTICAL NURSE OR MULTISTATE NURSING LICENSEQualificationsCompletion of a formal education program in practical nursing.Current state LPN licensure.Prior outpatient, quality, or population health experience strongly preferred.Experience with EMR's, data abstraction, patient outreach, or value-based care initiatives preferred.
