Your clinical expertise will help guide patients through complex health situations, improve outcomes, and promote long-term wellness in the community.Job specificationCandidate requirementsJob specificationConduct in-home assessments to monitor patient safety, identify health changes, and provide medication reviews and clinical coaching.Implement and maintain individualized care plans using digital tools for ongoing patient support.Offer practical resource referrals and evidence-based health guidance through our user-friendly app.Build meaningful relationships with caregivers to reduce stress and promote positive behavior changes.Accurately document all assessments, interventions, and outcomes in compliance with regulatory standards.Participate in additional care-related projects and perform other duties as assigned.
