Schedule: Full-Time, M-F 8am - 4:30pm.Location: This position serves home care patients in Minnetonka, Hopkins, Eden Prairie, Chanhassen, Mound, and Victoria, MN.Pay rate: Up to $75/hour Depending on Experience!*This is a temporary assignment expected to start ASAP and will last at least through June 2026 with the potential of it extending longer. What we offer you:Locally Owned and Operated BusinessOpportunity to work different locations/Diversity in clinic settingsWorking with Clinics and Hospitals throughout the 7-county area of Minneapolis/Saint PaulFree Education CoursesCompetitive Salary and BenefitsHealth CoverageDental CoveragePTO Accrual Based on Hours Worked What we ask of you:Assess physical, psychological, social and environmental needs of the patient and family.Integrate principles of development and disease process including co-morbidities in creating the plan of care.Maintain necessary records and use them appropriately for the care of patients, and demonstrate proficiency in the OASIS data set time points.Provide patient/family education including patients self care and care provided by family.Serve as a case manager for assigned patients and coordinate with all providers involved in patient care including RN, SW, home health aide and other rehab disciplines.Assist patients and families in resolving concerns or complaints of care or service, and provide direction and supervision of care by home health aides.Assess needs and refer to Adult Protection Services regarding status of vulnerable adults as necessary.Provide skilled clinical care including evaluation and treatment of patients, and the development/implementation of care plans. Anticipate the needs of the population served as well as the unique characteristics of the communities served.All other duties as assigned. Required Skills/Qualifications:Must have a current license with the Minnesota Board of Physical Therapy.Must have a Bachelors, Masters or Doctorate degree in Physical Therapy.At least one year of previous experience as a PT in home care settings.Must be comfortable/familiar with Medicare visits and have knowledge of OASIS Start of Care process, Must demonstrate knowledge of age related growth and development principles to provide appropriate service regarding primary diagnosis and multiple co-morbidities affecting care.Proficiency with communication and data processing equipment, clinical apparatus related to home care, electronic medical record system, OASIS evaluation and data set.Must have reliable transportation for client visits.
