Physical Therapist (Home Health)

We are seeking a highly motivated and skilled Physical Therapist. The Physical Therapist will play a crucial role in helping patients regain mobility, improve their quality of life, and achieve their rehabilitation goals.ESSENTIAL DUTIES AND RESPONSIBILITIES:Conduct thorough assessments of patients' physical conditions, including strength, range of motion, coordination, and balance.Develop and implement individualized treatment plans based on patients' needs and goals.Provide hands-on therapeutic interventions, including exercises, manual therapy, and specialized techniques.Educate patients and their families about the treatment process and home exercise programs.Monitor patients' progress, adjust treatment plans as necessary, and document accurately and timely.Collaborate with other healthcare professionals, such as physicians, nurses, and occupational therapists, to ensure comprehensive patient care.Maintain a safe and clean working environment, adhering to all infection control and safety protocols.QUALIFICATIONS:Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.Valid state licensure or eligibility to obtain licensure as a Physical Therapist.Experience in home healthcare as a Physical Therapist.Strong knowledge of anatomy, physiology, and physical therapy principles and techniques.Excellent communication and interpersonal skills, with the ability to build rapport with patients and collaborate with a multidisciplinary team.Demonstrated ability to work independently, prioritize tasks, and manage time effectively.CPR certification (BLS or equivalent).Home health care experience and experience with OASIS is required.Experience with Homecare Homebase is a plus!
