This role plays a critical part in ensuring appropriate reimbursement, quality resident care, and regulatory compliance. The MDS Nurse collaborates with the interdisciplinary team to assess resident needs, develop individualized care plans, and ensure documentation reflects the clinical status of each resident. This position may also provide direct resident care as needed and participate in the facility’s on-call rotation. Coordinate and complete MDS assessments (admission, quarterly, annual, significant change, and discharge) in a timely and accurate mannerEnsure compliance with CMS, state, and facility guidelines related to MDS and care planningInitiate, develop, and update individualized care plans based on comprehensive assessmentsCollaborate with interdisciplinary team members (nursing, therapy, dietary, social services, and providers) to gather and validate resident dataAccurately code MDS to reflect residents’ clinical conditions, treatments, and services providedMonitor Case Mix Index (CMI) and support optimization of reimbursement through proper documentation and codingParticipate in care plan meetings with residents and families as appropriateReview clinical documentation to ensure it supports MDS coding and regulatory complianceIdentify and communicate changes in resident condition to the appropriate team membersConduct audits and participate in quality assurance and performance improvement (QAPI) activitiesStay current with changes in MDS regulations, RAI Manual updates, and reimbursement guidelinesProvide education and guidance to nursing staff regarding documentation and MDS processesProvide direct bedside nursing care as needed to support staffing needs and ensure continuity of resident careAdditional Duties & Scheduling Expectations:Participate in the facility’s on-call rotation, approximately every 8th weekend, to support clinical operations and address urgent needsMaintain readiness to respond to staffing or clinical issues during on-call periodsDemonstrate flexibility in assisting with floor nursing responsibilities when requiredQualifications:Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standingPrevious experience in long-term care or skilled nursing preferredExperience with MDS/RAI process strongly preferredStrong assessment, critical thinking, and analytical skillsExcellent attention to detail and organizational abilitiesEffective communication and interpersonal skillsAbility to work independently and collaboratively within a team environmentQualificationsGraduate from a National League of Nursing (NLN) or American Association of Colleges of Nursing (AACN) accredited nursing program is preferred. Bachelor’s Degree in nursing preferred.Minimum of one-year experience working as licensed nurse with the senior population preferred. Specific services/positions may have additional education, training or experience requirements. Resident Assessment Coordinator-Certified (RAC-CT), management or supervisory experience preferred.Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required certifications and completes required continuous education units (CEU) and competency training.
