Requirements Experience 12 months of role experience is required with some in the last 12 months. Experience with Epic is preferred from any number of months. References Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role License Candidates must have a license Certifications BLS (Required) Vaccinations COVID vaccination required. Religious and medical declinations accepted. Flu vaccination required. Religious, medical, and personal declinations accepted. Additional Details: Outpatient Required Skills/Experience: Must be able to travel to multiple points of care. Required Credentials: Current unencumbered Oregon Physical Therapy license required. Healthcare Provider Level BLS required.
