Community Health Navigator

This position is partially or fully grant-funded. Employment beyond the term of the grant award may be subject to approval by Executive CouncilHere’s What You’ll Be DoingServes as a central point of contact for clients requesting assistance from the Community Health Department and as a liaison between the Medical/Community Health staff, Family Services and other tribal programs on behalf of clients needing assistance. Assesses client needs, including medical, emotional, psychosocial, and social determinants of health. Develops individualized care or navigation plans and collaborates with interdisciplinary teams both within FCP and outside organizations and healthcare systems.Facilitates communication between clients, families, internal and external healthcare providers, and other key resources.Helps to identify and address barriers to care such as transportation, housing, insurance, appointment availability, financial challenges or other cultural barriers. Documents all client visits into appropriate documentation systems following the FCPCHD documentation policies. Ensures confidentiality of all records and information.Coordinates referrals to internal departments and external community resources and follows up to ensure service delivery.Provides education and support around diagnosis, treatment options, and health system navigation. Advocates for client needs within the healthcare system and the community. Participates in Quality Improvement and Public Health Accreditation activities as requested.Attends meetings, trainings, events and in-services that may require occasional evening, overnight or weekend hours as assigned by supervisor.Here’s What You’ll Need To Be Successful Associate’s or Bachelor’s degree in social work, public health, nursing, human services, or other related fieldOther healthcare professionals considered with minimum of 2 years of experience in a clinical, hospital, or community health setting.Strong understanding of the healthcare system and community resources.Excellent communication, problem-solving, and organizational skills.Prior experience in care coordination, case management, or patient navigation preferred.Must have and maintain a valid driver’s license and be legible for coverage under the Tribe’s insurance.
