The CNA's will interact directly with patients, providing comfort, monitoring vital signs and assisting with daily living needs. Flexible hours with various shifts available.NURSING ASSISTANTJOBSUMMARYHe/She isresponsible for the care of the residents in a designated area of the facility.He/She isknowledgeable of the information as outlined on the Interdisciplinary Care Plan and follows the plan. He/She may provide restorativecare for each resident in the assigned group keeping the resident well-groomed, clean andassists to meet thepsychosocial needs. He/She assists to keep the resident care areas clean, ingood working order,and a homelike atmosphere for the resident. He/She communicates with the Licensed Staff and otherdepartment heads of the resident's needs. He/She reports all relevant observations to the LicensedStaff assigned to the unit in a timely manner. He/She documents information timely as assigned. He/She attends in-servicesand staff meetings as required.TheNursing Assistant is responsible to the Licensed Staff assigned to the specificunit.EDUCATIONMust beable to read, write, and follow oral instructions. High school education or equivalent is preferred. Must have current certification as requiredby state regulations.QUALIFICATIONS1. Hasthe ability to provide care to a group of residents assuring quality is provided atall time for this group as well as others in the facility.2. Maintainseffective interpersonal relationships.3. Is ofgood emotional, mental and physical health having sound judgment and high standards.4. Iswilling and capable of providing emergency care as needed for any resident in the facility.5. Is courteous in working withresidents, families, and employees.6. Maintainsa neat, well groomed appearance at all times. Keeps personal appearance appropriate forresident care.7. Hasa desire to work with the geriatric resident and their current and prospective needs.8. Mustbe in the facility ready to begin assigned duties on time and as scheduled.9. Mustbe willing to work preferred shifts as scheduled and assist on other shifts as needed to assure adequatestaffing to meet the needs of each resident.10. Musthave the ability to provide the care as outlined on the Interdisciplinary Care Plan and other assignmentsfor each resident in assigned group.11. Mustdemonstrate ability to function as a cooperative team member with each member of the NursingDepartment and other departments.12. Musthave ability to provide related bedside resident care services as per facility policy and procedures.13. Mustdisplay ability to adapt to on-the-job training and adhere to standards of the facility providingpersonal care services, assisting residents, caring for equipment and supplies anddeveloping good understanding of basis of aseptic and sterile techniques.14. Must beable and willing to function appropriately in a disaster or evacuation of thefacility.15. Mustdemonstrate ability to follow direction of supervising staff.16. Musthave a pre-employment and an annual Tuberculosis screening examination.NursingAssistant, Certified JobDescription Page 2RESPONSIBILITIESIt is tobe understood that the Nursing Assistant works by general, consistent, pre-arranged"Primary" group assignments as the basis for each shift worked, but must be able to accept changes andadapt to added assignment as the need arises. In addition, each NursingAssistant must work with each other to assure safe practices for each residentand staff are followed.1. Signsin per facility protocol.2. Checks residents and makes roundson assigned group noting significant issues for intervention. Begin planningtime to assure all resident's needs are met throughout the shift.3. Assists residents as indicated toprovide A.M., P.M. and daily care to assure residents are clean, dry,toileted, continent, well-groomed and cared for in the manner they deserve. Followsrestorative plan as outlined on the Interdisciplinary Care Plan. Provides for privacy anddignity at all times.4. Assistsresidents to meals. Assists residents to toilet prior to meals. Assists resident to wash hands and face,brush teeth, put on glasses, get to dining room and in good position prior to meal.5. Providesassistance and restorative eating as indicated. For those residents who are the most debilitated,consider their weakness in planning time for the whole group of residents.6. Aftermeal, assist resident to wash hands and face, return to their room and toilet as needed.7. Assists in dining areas as assigned. Offers assistance toresidents as needed. Offerssubstitutions for items not eaten well by any resident. Promotes atmosphere conducive to increasednutritional intake for each resident in the dining room.8. Forthose residents unable to go to the dining room, assists in service, assisting and supervising meal service as assigned.9. Assistsin transporting used trays to the kitchen following appropriate infection control policies andprocedures.10. Assistsresidents to plan their time to enable them to be well cared for as well as attend their desired functionsduring the shift. (i.e.activities, visitswith each other andtheir families, outings, meals, quiet times, etc.)11. Answerscall lights timely assisting with care as indicated. Keeps call lights available for the residents.12. Encouragesand motivates residents to participate in recreational activity of body and mind.13. Providesand keeps fresh water and fluids available for the residents at all times. Offers fluids at leastevery 2 hours or each time in the room, when appropriate.14. Timelyserves and provides assistance with between meal nourishments as indicated.15. Followsbath and whirlpool schedules as outlined for each resident. Do nail care, wash hair, shave, etc. to assure personal hygiene practices are always followed.16. Checksall residents frequently and at least every 2 hours assuring the resident is repositioned, turned,toileted, clean, safe, etc. Provides intervention as needed to provide a safe cleanenvironment.17. Checks all residents daily for condition of skin,reporting findings to Licensed Staffassigned to unit.18. Assists licensed staff when needed to position residents in needof treatments.19. Keepshand rolls and positioning devices in place as needed and as indicated for each resident.20. Assistsresidents to bed for naps or night sleep as needed. Provides a quiet restful and safe atmosphere whenresidents are resting.21. Takesand records vital parameters as assigned. Assists with weekly and monthlyweights as assigned.22. Cleansand properly arranges all bedroom furniture to include keeping the residents orderly, clean, andfollowing infection control guidelines.23. Follows Plan of Care.24. Assiststhe residents in keeping closets clean, clothing clean and in good repair,marked with their name, etc.26. Assistsresidents to keep bathrooms clean, following infection control and privacy practices.27. Changesresident's bed clothing to assure the bedding is clean, not wrinkled, and promotes good hygiene andcare practices.28. Followsappropriate infection control practices in working with the residents and equipment.29. Keepsaccurate records of intake and outputs and procedures throughout the shift for those residents whorequire such.30. Records information on the Nursing Care Records timely.31. KeepsLicensed Staff informed of pertinent information timely. Writes pertinent information on the GroupAssignment Sheet and Communication Books.32. Providesinformation accurately and timely for information to be added to the residents Interdisciplinary CarePlan.33. Cleansancillary rooms and equipment as assigned. Assists other members of the staff to keep each area of thefacility clean, orderly, safe, and a homelike atmosphere for the residents.34. Assistsother staff members as needed throughout the shift. Cooperates with other personnel to be sureresidents are of prime importance to all members of the staff.35. Assists new staff members in orientation as needed throughout theshift.36. Maintainssafe environment at all times, wiping up spills, working with housekeeping staff when availablepicking up articles as noted, etc.37. Givesreport to Licensed Staff and turn in completed Group Assignment sheet prior to leaving the unit to gooff duty.38. Performs other tasks as assigned.39. Attendsrequired staff in-services, meetings, orientation, and continuing education activitiesas required.40. Is aware ofand adheres to Patient's Bill of Rights and confidentiality of patientinformation including HIPAA regulations.41. Interacts withpatients and family members, co-workers, clinical and ancillary staff in anon-judgmental, supportive and calm manner.42. Is aware ofPatient Abuse Reporting Law.43.
