ESSENTIAL JOB DUTIES/RESPONSIBILITIESThe PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult population, taking full accountability for patient care and outcomes. The PCP will seek consultation from specialists when needed while remaining involved in the detailed care of the patient. It is expected that the PCP will engage with the hospitalist whenever the patient is hospitalized, regardless of the hospitalist’s affiliation with ChenMed. The PCP is responsible for assessment, diagnosis, treatment, management, education, health promotion, care coordination, and documentation for patients with acute and complex chronic health needs. The PCP leads a care team that includes a care promoter (medical assistant), care facilitator, and care coordinator for patients able to come to the office. For patients unable to attend the office—whether in hospital, SNF, LTC, or homebound—the PCP will engage with the transitional care team, case managers, acute and transitional‑care physicians, and other resources available in the market. The PCP also plays an active role in center management, covers for other providers when needed, and assists in recruiting patients and additional providers. The PCP supervises, collaborates with, participates with, or functions within a collaborative agreement with an Advanced Practice Practitioner (APP), remaining accountable for the APP’s actions while employed by the company. The PCP performs other duties as assigned and modified at the manager’s discretion.KNOWLEDGE, SKILLS & ABILITIESAvailability and Accessibility: Make themselves available to patients, offering walk‑in visits and timely responses to calls and messages.Service Orientation: Provide care that the PCP would want for a family member or for themselves at every interaction.Evidence‑Based Medicine: Stay updated on clinical, nutritional, and lifestyle‑based interventions to improve outcomes.Physician Leadership: Continuously develop and improve leadership skills for the benefit of patients, team, center, and company.Quality: Work with care teams to achieve high‑quality outcomes and quality measures, and seek continuous improvement.Influence: Compensately and compassionately influence patients, teams, and themselves to achieve optimal outcomes.Self‑Care: Maintain personal wellbeing physically, mentally, socially, and spiritually to care effectively for patients.Technology Proficiency: Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and other essential software.Travel: Ability and willingness to travel locally, and occasionally regionally or nationally for training or recruiting purposes.Availability: Flexible to work evenings, weekends, and/or holidays as needed.EDUCATION AND EXPERIENCE CRITERIAMD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.Must hold or be able to obtain an active State Medical License for the state(s) of assignment.Board certification in the relevant specialty preferred; board eligibility required.Maintain board certification through required MOC, CME or retake of exams.Active DEA number for schedule II–V controlled substances.Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross required within first 90 days of employment.PAY RANGE$221,141 – $315,915. The posted range represents the base hourly rate or base annual full‑time salary; final compensation may vary based on experience, education, location, and other factors.
