Home Health Care ServicesContinuing Care Retirement Communities and Assisted Living Facilities for the ElderlyResidential Intellectual and Developmental Disability, Mental Health, and Substance Abuse FacilitiesNursing Care Facilities (Skilled Nursing Facilities)Other Residential Care FacilitiesJob Responsibilities:Provides hands‐on personal care and assistance with activities of daily living, such as bathing, dressing, grooming, and toileting, in accordance with the patient's individualized plan of careDelivers care under the supervision of a Registered Nurse, following New York State Department of Health guidelines and all agency policies and proceduresEncourages and supports patients in performing tasks independently to promote self‐care and maintain dignityAssists with light housekeeping tasks to ensure a clean, safe, and organized home environment that supports the patient's well‐beingPrepares and serves meals using basic nutrition principles and modifies diet plans as directed by the Registered Nurse to meet the patient's health needsProvides care using infection prevention practices and reinforces infection control education given by nursing staffAids patients with prescribed rehabilitative exercises under the guidance of a Rehabilitation Therapist to support recovery and mobilityObserves the patient's physical, emotional, and behavioral condition and accurately records daily care activities and any significant changesCommunicates changes in the patient's condition immediately to the Registered Nurse or, if unavailable, to the Home Health Aide SupervisorParticipates as a valued member of the care team, offering feedback and suggestions to improve the quality and continuity of patient careCompletes all documentation in a timely, thorough, and accurate manner according to agency standards and regulatory requirementsProvides compassionate, respectful, and culturally sensitive care while maintaining patient privacy, dignity, and comfort in the home settingPosition Qualifications:Minimum Required:
High School Diploma or GEDandsuccessful completion of a Home Health Aide program within 12 months of hire,ORCurrent Home Health Aide Certification (no diploma/GED required if you're currently certified)Work Environment
Work is predominately in patient homes. Some work is performed in offices that require working in close quarters with other employees. The offices are air conditioned and heated. The job requires the following physical demands: Ability to work in a non‐sterile, unpredictable environment (furniture, other residents, icy walkways), including exposure to animals. Ability to stand and walk without restrictions. May require standing for 30 minutes or longer at one time. Ability to walk up flights of stairs carrying aide bag and supplies. Ability to bend, stoop, squat and twist. Ability to lift, push, pull patients a minimum of 50 lbs without restriction. Ability to independently transfer or assist patients to move, stand or walk in a safe manner. Travel between locations as required.Age of Patients Served
