Lead Care Coordinator

We want to ensure that care services are delivered appropriately and for the supervision of home care personnel.Responsibilities include:· Going down to the members home for initial orientation and monthly check ins.· Engage with the members. Offering companionship and emotional support.· Development of individualized care plan.· Monitor and document patient's condition.Must have:· Excellent communication skills and ability to sympathize with and understand member's needs.· CNA or other certified license in the health care field.· Valid driver's license.· Ensure compliance with HIPAA regulations while maintaining patient confidentiality.Job Type: Full-timePay: $25.00 - $30.00 per hourExpected hours: 40 per weekBenefits:Paid time offSchedule:Monday to FridayWork Location: In person
