Case Management Nurse

RN Case ManagerWe are seeking a compassionate and experienced RN Case Manager to coordinate patient care across the continuum and support safe, timely discharge planning. The RN Case Manager will assess patient needs, develop individualized care plans, coordinate post-acute services, collaborate with interdisciplinary teams, and advocate for patients and families to ensure quality outcomes and continuity of care.Key ResponsibilitiesAssess patients’ medical, psychosocial, functional, environmental, and financial needs.Develop, implement, monitor, and update comprehensive interdisciplinary care plans.Coordinate discharge planning and transitions of care from hospital to home, rehabilitation, skilled nursing, or other post-acute settings.Collaborate with physicians, nurses, therapists, social workers, and external agencies to ensure continuity of care.Facilitate communication between patients, families, caregivers, and healthcare providers.Evaluate medical necessity for treatments, procedures, and lengths of stay.Work with insurance providers to obtain authorizations and coordinate covered services.Identify and resolve discharge barriers, including financial, transportation, housing, or social concerns.Connect patients and families with community resources, support services, and healthcare programs.Educate patients and caregivers regarding diagnoses, treatment plans, medications, and self-care needs.Maintain accurate documentation of assessments, interventions, care plans, and outcomes.Participate in interdisciplinary rounds, care conferences, and quality improvement initiatives.Advocate for patient rights, preferences, and access to appropriate services.Ensure compliance with hospital policies, regulatory guidelines, and confidentiality standards.QualificationsEducationGraduate of an accredited Nursing, Social Work, or related healthcare program required.BSN or MSN preferred.Licensure & CertificationsActive Oregon RN License required.CRASE training required (or must obtain if assignment exceeds six months).BLS certification preferred.ExperiencePrevious acute care, home health, discharge planning, or case management experience required.Experience coordinating post-acute services and community resources preferred
