Hospice RN (Per Diem)

DescriptionThis on call/per diem position will travel to see patients in the Portland Metro and Clark County (Washington) areas.The Hospice Nurse under physician direction completes medical requirements while coordinating a whole person plan of care. Position is accountable for direct and indirect quality care for patients of all ages. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Position accepts professional development as an integral part of nursing practice. Coordinates patient care activities with other members of the health care team in a collegial relationship.
