Family Practice/Primary Care Physician Assistant

Medical Group that has been serving the local area for over 30 years and has a network of over 200+ physicians is seeking to add a Primary Care Physician Assistant to its team. The position is in a well established practice. As an employed PA, you will have full benefits, including Medical, Dental, Vision, Life, and a very generous 401k offering. PAY: $100,000 to $140,000/per year DOE Conduct Annual Wellness Exams (AWVs) in accordance with CMS guidelines Identify and accurately document chronic conditions for HCC risk adjustment Perform thorough medication reconciliation and patient education Collaborate with primary care providers and care coordinators to ensure continuity of care Utilize EMR systems to document findings and support coding accuracy Participate in quality improvement initiatives and compliance audits Required Skills: Minimum 1 year of clinical experience (HCC or primary care experience preferred) Strong knowledge of Medicare Advantage, HCC coding, and risk adjustment Proficient in EMR documentation and medical coding best practices Excellent communication and patient engagement skills Preferred Qualifications: CRC (Certified Risk Adjustment Coder) certification Bilingual (Spanish/English) Experience with in-home assessments or mobile care, willing to travel to local clinic sites within the Victorville, Apple Valley, Phelan, Hesperia.
