LPN Part TimeTryfacta is seeking a LPN Part Time for our client in Bloomfield NM 87413. This is a temporary contract assignment. If you meet the qualifications listed below and are interested, please apply now!Duration: 10 weeks to long termWork Schedule: 12 hrs. shift (Friday and Saturday)Guaranteed Hours: 24Pay rate: $35-$40/hr.Responsibilities for this position include, but are not limited to:Provide direct nursing care to residents in a long-term care settingAdminister medications and treatments while ensuring accurate MAR documentationMonitor and record vital signs, symptoms, and changes in conditionAssist with and oversee activities of daily living (ADLs)Perform wound care, dressing changes, and skin assessmentsSupport management of chronic conditions (e.g., diabetes, hypertension, dementia)Collaborate with RNs, physicians, and interdisciplinary teams on care plansRespond to emergencies, falls, and acute changes in resident statusMaintain timely and accurate documentation in EMR systems (e.g., PCC)Ensure compliance with infection control and safety protocolsProvide patient and family education regarding treatments and care plansSupervise and delegate tasks to CNAs and support staffQualification:Active Licensed Practical Nurse (LPN) license (State of practice)Minimum 1 year of Long-Term Care (LTC) / Skilled Nursing experience preferredCurrent BLS (Basic Life Support) certification.#HLT123 Tryfacta is an Equal Opportunity/Affirmative Action Employer.
