Reviews patient history, physical diagnostics, and laboratory data, and reports abnormal results to the physician.Implements care plans through direct patient care, coordination, delegation, and supervision of healthcare team activities.Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments in various home situations.Uses motivational interviewing and health coaching techniques to engage stakeholders in care management.
