SummaryThe Social Worker - Intake Coordinator is assigned to work with special patient populations and highly-complex health or mental health problems requiring expert level psychosocial interventions - the following criteria must be met when determining the grade of candidatesSenior Social Worker - GS-12 Experience/EducationThe candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which - one year must be equivalent to the GS-11 grade levelSenior social workers have experience that demonstrates possession of advanced practice skills and judgmentSenior social workers are experts in their specialized area of practiceSenior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialtyLicensure/CertificationSenior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination - unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California - which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensureDemonstrated Knowledge - or a national VHA referral center - such as a national Center for Post-Traumatic Stress Disorder or a national Transplant Center - or other program areas of equivalent scope and complexityThe senior social worker may be assigned administrative responsibility for clinical program development and is accountable for clinical program effectiveness and modification of service patternsAssignments include clinical settings where they have limited access to onsite supervision such as CBOCs or satellite outpatient clinicsThe senior social worker collaborates with the other members of the treatment team in the provision of comprehensive health care services to Veterans - ensures equity of access - service - and benefits to this population - ensures the care provided is of the highest qualityThe senior social worker provides leadership - direction - orientation - coaching - in-service training - staff development - and continuing education programs for assigned social work staffThey serve on committees - work groups - and task forces at the facility - VISN and national level - but is not limited to Maintains knowledge of Veterans benefits and services - community resources and process for making appropriate referrals to community and other governmental programsCoordinates of care and discharge planning for Veterans who are inpatient at hospitals in the community and/or inpatient at other Veteran hospitalsDevelops a treatment plan with the Veteran based on this assessment and develop the treatment plan that includes measurable time limited treatment goalsMust actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventionsWorks independently with Veterans and their families who are experiencing a wide range of complicated medical - psychiatric - emotional - behavioral and psychosocial problemsFacilitates biopsychosocial assessments - providing solution focused and short term counseling with Veterans and/or their families - and must complete thorough assessments to determine the treatment needs and recommendations for careCompletes independently thorough psychosocial assessments to determine the functioning and needs of Veterans and/or their familiesCoordinates screening of Veterans for specific programs such as PTSD - ADATP - PRRTP - CNH - VIST - HHA - OEF/OIF and provides ongoing treatment and documentationResponsible for providing education on advance health care planning and facilitates psychoeducational and support groups on a variety of topics for Veteran and their familiesAbility to expand clinical knowledge in the profession - provide consultation and guidance to colleagues - role model effective social work practice skills - teach or provide orientation to less experienced Social Workers - develop innovations in practice interventions - and provide clinical supervision for social work licensure or certificationKnowledge of medical center policies and procedures - VHA and VBA regulations - as well as a knowledge base of mandatory reporting requirements and procedures in the state their CBOC is locatedReceives and completes consults from interdisciplinary teams and other Social Workers on complex difficult cases using advance practice skills and expertiseIncumbent provides in person and/or utilize telehealth services for individual - group - and family psychotherapy and advanced level case management interventions used in the treatment of Veterans with poly-trauma injuries - spinal cord injuries - traumatic brain injuries - visual impairment - post-traumatic stress disorder - addictions - and other serious disorders Must possess a working knowledge and experience in use of medical and mental health diagnosis - disabilities and treatment procedures - including acute - chronic and traumatic illness - common medications and their effects/side effects and medical terminologyDocuments independently and in accordance with VHA DIRECTIVE 2011-025 - that workload closeout must be transmitted no later than 7-calendar days from the date of the outpatient care encounterThis includes - documentation in CPRS electronic medical records - including psychosocial assessments - diagnostics - treatment - progress notes - follow-up and referralsInterviews and conducts psychosocial assessments to establish Veteran's treatment needs - presenting problems - and DSM V diagnosesassesses high risk factors - acuity and need for services - at times for walk-ins and unscheduled visits and interprets and explains VA treatment and benefit programsMust provide consultation and education to Veterans and their families regarding community resources - VA benefits and specialty programs - and advance directivesThis includes the incumbent's knowledge of the process for accessing and/or coordinating community-based services - including information and referral for additional services from other VA programs -
