Where Fun is Encouraged!Job Responsibilities:As a Music Therapist at Custom Home Health, you'll be at the forefront of providing tailored care to patients and their families in the comfort of their homes. Your duties will include:Conducting thorough assessments of music therapy needs for patients and their familiesCollaborating with interdisciplinary teams to prioritize interventions and referralsMeticulously documenting all care provided in adherence to hospice documentation standardsEducating hospice staff and the community on the impactful role of Music TherapyOffering bereavement support to families when deemed appropriateFacilitating support groups as needed and ensuring efficient resource utilizationFulfilling additional duties as assigned with enthusiasm and dedicationPreferred Qualifications:Bachelor's degree or equivalent in Music TherapyState licensure as a Board-Certified Music Therapist (MT-BC)A minimum of two years' experience in music therapy, with a preference for community/hospice experienceDeep understanding of hospice philosophy, focusing on comfort and maintaining care in the patient's residence for as long as possible
