Peer Supportive Housing Case Manager Job Locations US-NY-Rochester Category Case/Care Management Position Type Full-Time Work Arrangement Onsite ID 2026-6709OverviewHelio Health is looking for a PeerCase Manager to support its mission of promoting recovery from the effects of substance use and mental health disorders and other health issues. This role is vital to the management and coordination of services to patients/residents participating in our low barrier OMH homeless scattered site supportive housing units. This case manager would support individuals in maintaining housing, navigating health and human services as well as skill development. Provide in-community, in-person support at least 4 times per month upon entry to the program. PCM will work with the individual, in collaboration with collateral supports and services to develop and execute an ISP, with goals and objectives that focus on housing retention, community integration and recovery, reviewing and updating the plan on a quarterly basis.
