RN Patient Transition Director

ESSENTIAL DUTIES AND RESPONSIBILITIESInclude the following. Other duties may be assigned.Develops, implements, and coordinates community outreach programs designed to explain and promote the service businesses of Affinis Hospice, its member organizations, and their programs and services to the community; utilizes various media aimed at informing specific targeted groups and/or populationsCollaborates with member organizations to establish and maintain community outreach education and community-based programs;Supports the implementation of mystery shopper programs and other initiatives that enhance the Service Excellence program; measure and track program outcomesSupports service businesses by speaking with community groups to explain the various services and activities available through the community outreach programMay support service businesses through the development of community-based health initiatives aimed at the prevention of disease and the promotion of health; may oversee and participate in specific community events such as health screenings, health fairs, and career days. May participate in the development and support of effective volunteer recruitment programsIdentifies new outreach opportunities through ongoing research and implementationSKILLS AND ABILITIESKnowledge of community relations, volunteer and social service programsKnowledge of marketing strategies and techniquesExcellent customer service, interpersonal and oral/written communication skillsKnowledge of personal computers and the ability to operate Microsoft applications to include Word, Excel, Power Point, and OutlookAbility to multi-task and function with little directionAbility to effectively communicate and present information in a clear and concise manner, both orally and in writingAbility to establish and maintain effective working relationships with the services businesses administrators, associates, patients and families, the general public, and volunteersMINIMUM QUALIFICATIONSMust have Registered Nurse License and be in good standing with the State of GeorgiaCHPN Certification PreferredExperience in Social Work, Marketing, or related field; preferred minimum one year experience
